Prebichol (Obeticholic Acid) is prescribed for Primary Biliary Cholangitis (PBC) in the following scenarios:
- In combination with ursodeoxycholic acid (UDCA) for adults who have not responded adequately to UDCA after at least one year of use.

Obeticholic acid is a Farnesoid X receptor (FXR) agonist. FXR regulates key genes involved in bile acid synthesis, lipid metabolism, and glucose control. By activating FXR, obeticholic acid reduces bile acid production in the liver and enhances bile acid transport, reducing liver exposure to bile acids.

✔️ Adult Dosage
- Noncirrhotic or compensated Child-Pugh class A: Start with 5 mg daily. If there is no adequate response after 3 months and the medication is well-tolerated, the dose may be increased to 10 mg daily. The maximum dose is 10 mg daily.
- Child-Pugh class B or C, or previous decompensation event: Start with 5 mg once weekly. If there is no adequate response after 3 months, and it is well-tolerated, increase to 5 mg twice weekly (at least 3 days apart). Based on response and tolerability, the dose may be titrated up to 10 mg twice weekly (at least 3 days apart).

Take FXR 10MG as advised by your physician. Swallow the medicine with a glass of water. Do not crush or chew the medicine. Your doctor will decide the correct dose and duration for you depending on your age, body weight, and disease condition.

- warfarin (used to prevent and manage blood clots)

Prebichol should not be used in patients with complete biliary obstruction.

- Hepatic decompensation and failure: In patients with advanced cirrhosis (Child-Pugh class B or C), the use of obeticholic acid at higher doses than recommended (e.g., more frequently than once weekly) may lead to serious liver complications, including fatal outcomes. Monitoring liver enzymes and signs of hepatic decompensation is crucial.
- Lactation: It is unknown whether obeticholic acid is excreted in human breast milk. Careful consideration of the benefits of breastfeeding versus potential risks to the infant should be made.
